Most causes of neck pain are considered “non-specific” or “mechanical”, meaning that there is no known serious underlying pathology. In other words, the pain you are feeling is coming from the function of your spine and the supporting anatomy around your spine, such as muscles, joints and nerves.

Vertebral subluxation — even a minor misalignment in the cervical vertebrae (C1 to C7) can disrupt normal joint movement and irritate the spinal nerves running alongside them.
Disc degeneration is common in the lower cervical spine, particularly C4–C7. As disc space narrows, nearby spinal nerves can become compressed, producing the arm pain or tingling that many people mistake for a shoulder problem.
Facet joint irritation occurs when the small joints at the back of each vertebra become inflamed — from poor posture, repetitive strain, or a sudden trauma like whiplash.
Muscle spasm is usually a consequence, not a cause. Your neck muscles go into protective spasm when the underlying structure is unstable. Treating only the spasm — without addressing the structure — is why so many people find temporary relief, then relapse.
